IIRC, there was something wrong with one of Shaq’s hands or wrists, which affected his FT shooting. Perhaps that would have affected underhand FTs. Perhaps he tried it unsuccessfully in practice.
Hmm, Wikipedia says he was better in practice, so issue may have been mental
