I’m either reading something wrong, or the author is a liar. I went to the Lancet, and here’s what they said:

“Viral loads of breakthrough Delta variant infection cases were 251 times higher than those of cases infected with old strains detected between March-April 2020.”

It seems to me that they are comparing different Covid strains. But this Dr. McCullaugh is comparing vaccinated and unvaccinated people. The abstract does not mention unvaccinated people at all.

What am I missing?

I agree the wording is confusing and thus misleading. It seems he is comparing viral load of Delta variant cases to viral load of earlier “pre-vaccine” strains. Here’s the quote:

The conclusions of the Chau paper support the warnings by leading medical experts that the partial, non-sterilizing immunity from the three notoriously “leaky” COVID-19 vaccines allow carriage of 251 times the viral load of SARS-CoV-2 as compared to samples from the pre-vaccination era in 2020.
