I'll just address this one piece real quick because this is easy. Deaths are counted as being from COVID-19 when COVID-19 is listed by the doctor or medical examiner in the last line of Section 32 Part I of a US Standard Certificate of Death. Part 1 of Section 32 requires an ordered list - with onset intervals - for each of the morbid events leading directly to death. It starts with the Underlying Cause of Death (UCoD) and ends with the immediate cause of death.

In between each line, it says "due to". In other words, Event C, due to Event B, due to Event A. For example, hypoxemia (immediate cause of death) due to ARDS due to COVID-19. In that sequence, COVID-19 infection caused ARDS, which caused hypoxemia, which caused death. And that's it. That's all that goes into Part 1: only the events in the actual chain of events that lead directly to death. Other conditions which may have complicated the patient's survival go into Part II of Section 32. That could include things like diabetes, hypertension, asthma, and so on.

Those events have to make sense in their sequence. You don't put hypoxemia due to ARDS due to diabetes due to COVID-19 because diabetes isn't caused by COVID-19 and ARDS isn't caused by diabetes. And you don't put just COVID-19 because that doesn't kill the patient; something else that COVID-19 causes does. And you don't put ARDS due to COVID-19 due to cancer because cancer doesn't cause COVID-19.

The only deaths counted are ones where COVID-19 is listed as the UCoD. And with very few very rare exceptions, the death certificate's chain of events in Section 32 Part I are going to make sense because doctors and medical examiners fill them out as a matter of routine. They know what to do. And if they don't, CDC gives plenty of both general guidance and guidance specific to recording deaths from COVID-19 (https://www.cdc.gov/nchs/data/nvss/vsrg/vsrg03-508.pdf)

Take NYS deaths, look for a sudden fall off during the last 18 months or so in deaths with failure to thrive listed as the immediate cause of death.

If there is one, and I’ll bet there is,, Houston, we have a problem.
