Working in industry, I do believe. someone wanted a bonus and cut back up and failsafes out. Did you ever work for ABN AMRO or LaSalle Bank? (now defunct)
That's exactly what Senior Management did there when bonuses were in jeopardy of being cut. I.T. Budgets got slashed to the bone and then nothing got done. Not even the critical work.

But I banked with LaSalle.
Thought about taking them to court for some of the games they did with automatic deposits being held up for weeks.
