You can download the PDF file of the card. I have no idea how much they track on a computer the info in the card. I.e. Does some DB say Jon Smith got the J&J vax, lot number 75F43D on 4/3/21 or is all that info just on the cards?
They typed in my info to the computer each time I got a shot along with writing it on my card. I have no idea if anyone outside the medical establishment can get the database info because of HIPAA rules. But without the crosschecking it would be pretty easy to just print a cardstock copy of the blank form and have a couple friends fill it out by hand (to avoid having the same handwriting on every line).
Now, will Duke require a matching photo ID - or is that too racist?
