Wasn't it a United DC-10 out of O'Hare that crashed upon takeoff... when an engine fell from the wing, due to slipshod maintenance procedures? I have it on hearsay from another former United employee that one of the mechanics responsible for that later committed suicide.
That was American Airlines. And it was an entire practice that was on there to cheapen out on moving engines around. Never heard about any suicides, but might’ve happened.

Although a United employee could have told you, the TV and newspaper coverage was intense. Improper procedure when changing the engine cause the engine to become detached from the wing mount right after takeoff.

What I remember was that in a day or so I was flying out of Adak Alaska and would be taking a DC-10 for a transcontinental leg into Dulles.
I was a bit concerned.
