I don't think you get it. Not having the vaccine would very likely be the disability itself. Since it's a disability, you can't discriminate against those people. A law professor writes about the legal hurdles to these possible vaccine 'passports' below.

If nothing else, it will be a “jobs” program for lawyers in 50 states...

All I can say is good luck convincing a judge that your unwillingness to get a vaccination is a disability.

A law professor writes about the legal hurdles to these possible vaccine 'passports' below.

He wrote that article in May of last year when the vaccines weren't really on the horizon. In it he says:

"I’m concerned about the potential legal and ethical ramifications of immunity passports that aren’t based on vaccinations."

In the article he explicitly says vaccination based "passports" are OK (legally).
