If the ballot wasn’t filled in properly than it doesn’t count. No poll worker should be doing anything with the ballots except opening the envelope, unfolding it and feeding it into the machine.

Exactly right. Some people only vote certain things on their ballot I did just that on Saturday when I voted. I did not vote my whole ballot.

True. Vote for no more than three means what it says. What if the voter only wanted to vote for two candidates, or zero?
