Just who decides what ballots & time frame ballots are accepted in a Federal Nationwide election?

I believe ballots are verified,counted and the totals certified locally by the elections boards in each county.

But just who decides what ballots, what ballot design, and what’s acceptable?
Is it state legislatures? Or just a State Governor?????
