I think HCQ (and probably AZM, but there are problems with antibiotic and idiots overusing them, resulting in resistant bugs) should be sold OTC, like it is in some other countries. It’s actually safer than aspirin and ibuprofen.
BUT, even if some miracle occurs and they do allow it OTC, don’t expect to find any. A few Howard Hughes types, hoarders, and specuators will empty the shelves in minutes. And then you’ll get some idiot taking the whole bottle at once and croaking. Guaranteed. Because Idiocracy isn’t a comedy movie. It’s a documentary.
They’ll have to have some kind of controls to prevent hoarding. IMO, it’s would be better allow doctors to prescribe it freely, but on an one-per-customer basis.

No need for the test. When I was in Korea, in the 70s, hydroxychloroquine was on the mess hall tables right next to the salt and pepper. No physician administered a test. No one keeled over from taking. In fact, there was a small sign that told how many to take and how often. I know of no resulting problems.
The phenylephrine decongestant you can buy OTC has more cardio risk than HCQ.
They had to get rid of pseudoephedrine because of the War on Drugs and that has less cardio risk than phenylephrine. Another win for Team Drugs and another loss for john Q public.
