Even as hospitals and governors raise the alarm about a shortage of ventilators, some critical care physicians are questioning the widespread use of the breathing machines for Covid-19 patients, saying that large numbers of patients could instead be treated with less intensive respiratory support.
If the iconoclasts are right, putting coronavirus patients on ventilators could be of little benefit to many and even harmful to some.
Whats driving this reassessment is a baffling observation about Covid-19: Many patients have blood oxygen levels so low they should be dead. But theyre not gasping for air, their hearts arent racing, and their brains show no signs of blinking off from lack of oxygen.
That is making critical care physicians suspect that blood levels of oxygen, which for decades have driven decisions about breathing support for patients with pneumonia and acute respiratory distress, might be misleading them about how to care for those with Covid-19. In particular, more and more are concerned about the use of intubation and mechanical ventilators. They argue that more patients could receive simpler, noninvasive respiratory support, such as the breathing masks used in sleep apnea, at least to start with and maybe for the duration of the illness.
The patients in front of me are unlike any Ive ever seen, Kyle-Sidell told Medscape about those he cared for in a hard-hit Brooklyn hospital. They looked a lot more like they had altitude sickness than pneumonia.

Mechanical ventilation is a life-saving intervention in critically ill patients with respiratory failure due to acute respiratory distress syndrome (ARDS). Pathological lung over-distension caused by mechanical ventilation at high tidal volumes, however, transmits pathologic mechanical stress to alveolar epithelium and pulmonary vasculature, leading to barrier dysfunction. Thus, mechanical ventilation also creates excessive mechanical stress that directly augments lung injury in established ARDS, a syndrome known as ventilator-induced lung injury (VILI).

I was chatting online with my HS girlfriend. More than 40 years later, we are still very close. She's a cardiac RN. She's been working in the local hospital's ICU (in NJ) as a "liaison" between patients' families and the ICU. What she does is set up iPhone Facetime (or other) sessions with the families so that they can "see" the patients before they die.
I asked what she thinks the survival rate is for those put on ventilators. She said 15 to 20 percent.

This could be why malaria treatments seem to cause positive response as they similarly intercept virus trying to bind to the ACE2 receptor for payload delivery.
Also, traditional viral prophylactics like zinc, and anti-oxidants agents like vitamin C would theoretically have benefit, as they would against a common cold, since that is essentially what a coronavirus is - a cold where instead of sneezes and wheezes, the side-effects are oxygen deprivation, with domino effects of cytokine storm, organ failure, and death.
Anti-viral agents and traditional cold remedies will help "flatten the curve" of virus buildup in the blood giving the body a chance to develop immunity before it stops operating from oxygen starvation. Hydroxychloroquine will aid with more advanced infection by directly interfering with the ability of virus to hijack cells; zinc inhibits infected cell replication. Vitamin C and other anti-oxidants will combat free radicals caused by virus dissociation of iron.
A corona vaccine is hugely difficult due to the rapid mutation (are you immune for this year's cold or flu because you had one in the past? no?) Just stop people from dying from the symptoms before the body's natural ability to recognize and combat the disease runs its course, and that can be done now if this theory is correct.
The push for a vaccine, home quarantine, tracking and control, digital certificates, and anything else that Bill Gates makes money from, is about something other than the health of the human species.

Their lungs were failing. In those cases the ventilator merely prolongs life.
For COVID patients Trump said the number was too bad to publish.

That may have been it in your case and I am glad you got better. That is not the typical design of an icu
And that is why we have ARDSnet and other tools to adjust ventilators to prevent the kind of damage this article is speaking of. As I said we are getting better all the time with mechincal ventilation and we do. It cause much barotrauma like we used to.
It goes both ways. Inhalation of nasally derived nitric oxide modulates pulmonary function in humans.
Apparently leeches are indicated in some cases these days.
Jane I’m not sure where this idea there aren’t enough ventilators NOW is coming from.........In my state there’s plenty enough currently. And nearby counties without any covid patients even have them.
Are Dr’s just trying to get as many as they can just in case? In fact there appears to be more people on NON-covid ventilators than there are those with Covid.
