I think the point is that Fauci has emphatically described the evidence supporting clinical use of these agents as “anecdotal” and stated that full clinical trials are needed to guide pharmacotherapy. Based on my understanding of the literature, that characterization struck me as inaccurate and unfortunate. I don’t know whether Dr. Fauci sees and treats patients, but his statement appears to ignore the realities of routine off-label drug therapy.
As to your 2nd point, the information in the French group’s manuscript cited above—recently submitted and publicly posted—may help. I grant you it has not been accepted for publication at this moment, but my impression (based on some years of experience as a peer reviewer and editorial board member)is that, given the topic, acceptance is likely. They report treating patients who presented with a wide range of disease, along with some outcomes data. You might find it to be useful data.
