The whole purpose of Direct Observation is to have continuous 24/7 blocks 10 min shifts whereas the guard will log every 10 mins what the inmate was doing since the last entry or immediately if the status changes for example a log entry could be “12:00 - inmate still sleeping etc. But since it is not a “check” every 10 mins let’s assume at 12:03 the inmate wakes up and starts reading. You don’t wait until 12:10 to document that the inmate is awake and reading. At 12:03 you document that change. Then if the inmate is still READING at 12:13 your next log entry would be due no later than 12:13 etc.
