In business over the years I saw revolving door and turnover, and at break neck pace in some instances.
Experience taught me a few things in this regard.
What it tended to point up was one thing or the other, but usually nothing else.
A) Either the executive was hiring total failures (meaning they were good on the surface but turned out disloyal or incompetents) for those positions and therefore his/her own due-diligence and reading of people, and accordingly his/her leadership had to be called into question.
Or
B) He/she was actually hiring good staff but for some reason was either frustrating them, not soliciting/respecting/taking their advice, or not motivating them or appreciating them.
As I said, it was usually never another explanation than these two.
The last I heard was, the buck stops here (Oval Office).
But maybe thats changed.
