Some thoughts:
1) I am not sure if Trump even brought the subject of extra-judicial killings up. I don’t think he did, that is why Duterte likes him.
2) Duterte’s standard response to this accusation (foreigners or domestic critics ) is to curse and say that people do not understand the gravity of the institutional corruption and immensity of the problem he faces.
3) His defenders say that the so-called Extra-judicial killings are drug lords whacking their own drug dealers and small time pushers to prevent them from squealing, making it look like the cops did it.
4) Duterte himself does not provide any assurances as he boasts publicly of having killed people ( even when he was a teen ) , even comparing himself to Hitler.
His defenders tell us that he is saying this in jest.
So, there you go....

Even Saudi Arabia gives you a trial before cutting your head off. As does Iran before hanging.
They even get something that looks like a trial in Norklandia.
Baloney. He isn’t murdering people. They are going in after dope gangs that are nothing like we see here. If they resist, they are shot.
Also think about who has lined up against him, and who is “telling” you what he is doing. That should be the bad feeling we get...
He’s also admitted to killing a man over a ‘look’ when he was a teenager besides all the murders he’s responsible for when he was a governor. I don’t trust the government to decide who lives and who dies (or who lives and who is murdered) but a lot of people approve of him. Besides low level drug users and pushers he’s targeted his opponents both with death and threats of death. The police can kill also anyone who has offended them or who they want to steal from with impunity.
Being popular with the locals doesn’t matter much to me either. A long list of very bad leaders were popular.
He’s been whacking people his whole life, he’s not young, and he has no real base of support in the Philippine oligarchy or institutions.
The biggest danger is the opening he’s always flirting on giving the Marcos clan. Though I suspect this is just to keep up the political tension between the clans.
Don't let it trouble you. Duterte has years of whacking bad people ahead. There's no worry that he's going to run out of bad hombres anytime soon. His muslim problem is enough to keep death squads occupied for a long, long time.

You’re confusing leftist murder cycles with rightwing order restoring cycles. Both involve killing, but the right-wing ones come to an end when order is restored why left-wing ones just keep getting crazier and crazier.
I think the answer is the sovereign state determines what it must do to survive and thrive. If Duterte takes it to an unreasonable level, history will let him know.
