My guess is that he shot himself in the mouth

I thought that too from looking at the photo.

But one of the cops who rushed the room said on 60 minutes he looked at the body and saw no obvious head wound. He said he did see blood coming out of his mouth.

If he had used a .410 shot shell with small (#6, #7-1/2, or #12) shot, there probably would have been no exit would. And the same would have been true if he'd used a "pre-fragmnented" round like a "Glaser Safety Slug"

in the .45 chambering.

However, in either of those cases, there would have been massive brain tissue disruption -- enough that I can't imagine any competent pathologist stating categorically that there was no brain abnormality.

This just emphasizes that there is still a heck of a lot we don't know about this case...
