Yeah but there aren’t many great ones in burbs. Those that are are usually part of the system of the greats (like my local hospital - only acquired by Hopkins in the last 20 years). And the greats are teaching hospitals - almost always “in town”.

For really big problems, I want a borderline-Asperger's nerd who doesn't know there is a person connected to the condition, but knows everything in the world about that condition.

That is a good answer.Long,long ago when my mother was very sick (and,as it turns out,was only a few months from death) she had brain surgery to remove a cancerous tumor.It was performed by a top neurosurgeon at one of Boston's major hospitals.We all hated him because he had the "bedside manor" of an ill tempered robot.But when my mother would curse him we reminded her that you pay specialists like him for his skills...not his personality.
