The ValueJet crash was caused by Oxygen generators being shipped, not installed.
There were a bunch of them being shipped together and they somehow started a fire. I know LOX isn't generated in flight (LOX is made through the liquefaction of air that takes a lot of electricity, I know I used to buy it for the company for whom I worked) but the O2 generators do make it by a chemical reaction that makes heat.

The T-45 has been around since the early 90’s, and has always had OBOGs. The Navy has had some OBOGs issues, but I wonder if they have gotten worse recently. There is still little excuse for not having a warning system to warn when the OBOGs starts making poison gas.

haha! not much oxygen at 25 angels. Part of flight training used to be hypoxia chamber and putting together a child’s puzzle with mask (o2) off. It became impossible very quickly translating thought to mechanical action. After a few minutes it was even hard to understand, “if you don’t put on your mask now you will die.” Some had to be assisted.

Yes, it seems like a simple sensor in the out line (perhaps also taking in a feed from the altimeter (since it seems to vary the o2 % by altitude) with a buzzer and a flashing LED would address any problems. Maybe $5 worth of hardware and sell it to the Pentagon for $500 a plane.
