Those whose income is low enough, get it free. Probably the same people who get huge earned income credit refunds or whatever, just by keeping their income low and have lots of children.

I'd like to know exactly what she got, though. A very low-income friend of mine also signed up for the cheapest premium plan, and he still can't afford to use it. (The "cheaper" the premium, the heftier the co-pays.)
