What HRC had and has had in the past is (in this case) a generalized seizure episode. She became unconscious and so was unable to control her muscle movements, therefore would have fallen without assistance.
There are several things to consider here as to where she may have been transported. The van could have been equipped as an (unmarked) ambulance, perhaps even specially equipped to deal with a seizure patient. Chelsea's apartment may have had an area equipped similar to an emergency room, knowing that an episode such as this might very well have been forthcoming because they had occurred several times in the past. Add to that the fact that she was escorted into the van by the heavy-set black man believed, from previous discussion, to be a neurologist carrying a Valium syringe in his pocket, which would likely be effective in stopping the seizure. What you have, if the Valium works, is an end run around what would be, for 99.9% of patients with these symptoms, an obligatory trip to the ER.
That being said, we can't be sure that she was indeed taken to Chelsea's apartment as her campaign stated, what with the history of lies coming from that source. She might have gone to a secret, private ER open to her by previous arrangement. In either event, it is very plausible that she stopped seizing and recovered consciousness from the administration of the Valium. Even after the recovery, though, she would have no memory of what happened.
As for the underlying cause of these seizure episodes, one would guess that she's had appropriate work-up in the past, and whatever is causing the seizures (if they know) is not for public consumption.

I know I get dizzy, disorientated and need a arm to steady me and I’m weak for hours, I don’t go stiff like with a seizure. Not even when I had the bad 1st vertigo attack 5 yrs ago. It’s why I schedule any fasting test for first thing in the AM, but some take hours like a full Cardio work up and you are just shoved out the door dehydrated. Same with the Gallbladder HIDA test. When the Hematologist took a pint of blood to reduce platelet count, he gave me a pint of IV solution, but I was still quite sick for 2 days.
