In Ford's case, that was because in 1976, a significant number of states did not hold primaries. It was a lot harder to get a majority then. I believe Ford had won the majority of delegates that had been available in primaries and caucuses. Reagan had a lot of support in the grassroots delegations, and in those days, the party elites didn't control the process the way they do now. Bush learned his lesson in 1980, and he set up a system in 1989 that made sure that he and his buddies would never allow a Reagan to win ever again. And that system has worked perfectly. Bush, Bush, Dole, Bush, Bush, McCain, Romney. And we were supposed to get a Bush this time.
Cruz was ok with them, he was supposed to be this years Fred Thompson or Newt, but Trump blew the whole process to kingdom. And now, as a result, Cruz is still standing instead of nursing his wounds from the beating he would have received absent Trump in the race.

Not going to be Ryan. He needs to be running and have won at least eight states. The "8 states" requirement is only for the first ballot. If Trump doesn't win on the first ballot, it will be Ryan on the 2nd or 3rd.
