One fairly well known is a massive potassium dose. The body releases potassium into the blood after a heart attack, so it is HARD to detect.
The other is a drug. Don’t recall the name, but it’s not commonly available to say the least. It degrades within an few hours, so without a quick tox screen, it cannot be detected.
I’m sure there are others.
All of that said, I’ll reiterate that sometimes heart attacks just happen.

I think its Succinylcholine which is also a paralytic which makes it easier for the assassin to kill his target rendering them completely defenseless while they are paralyzed. Could be wrong but a toxicology screen must be done for this one drug because it will be overlooked in most screenings that are commonly used.
