We (myself included) are assuming the women are unwilling victims. Except for his wife, all we know is that 20 some odd women had their genitals surgically modified.

This is Africa, the land of clitorectomies, (all cultures are equal, doncha know) these women may well have chosen to have the procedure done by a man with medical training, sterile procedures and anesthetics over the local imam with a rusty razor blade.
