A DD-214 is a DD-214. Officers and enlisted alike are issued the form when they are discharged. It is simply titled “Armed Forces of the United States Report of Transfer or Discharge”
Mine has “NA” for Selective Service Number, SSBoard and date inducted. I was AFROTC, commissioned at graduation.
Keep it in a safe place.
