Maybe the flight crew just need to put a few more quarters in the slot... you know, like the DIY car washes?

On the Flyertalk website someone speculated that they lost one engine and the crew tried to restart the wrong engine, causing both to be shut down. I believe this is what caused the crash of the Taiwan airplane that we saw on video narrowly missing a bridge. Pretty scary if true.
