That was a DRT shot, liver hits usually pull the plug in less than a minute.
That was my first thought.
The only way I can see a bullet hitting all three organs, without a ricochet off a bone is to to enter from the lower left aiming up toward the right side of the neck.

Heart spleen and liver. Downward trajectory. Either he was diving or falling when that shot hit. It could have ricocheted off a bone and turned downward. Was he diving over a car or fence? Not a lot of details.
There are a lot of shootings where the victim is hit and subsequent shots as the body falls or turns have an upward or downward path through the body. Coroners can figure if the vic is twisting as they are shot and if they are twisting toward or away from the shooter. Cool stuff, if you can find the right books on the subject.
