My guess is the car accident in 1982 in Hawaii which was Bari Malik Shabazz...son of Malcolm X. He was ordered to court for driving without a license, and when he did not show a warrant for his arrest was issued. No one knew what happened to him. He was never found, In 1994 someone reported him dead, but no death certificate.
Later after many years, someone got that warrant dropped. No one ever insisted on a death certificate, and you can find all this online.
