As far as I can tell it was a kind of tongue-in-cheek nickname based on 50’s American westerns whose characters had names like “Pecos Bill” Or “Amarillo Pete”. Elton is the name of a small town in England, hence “Elton John”. His real name is Reginald Dwight.

The name “Elton John” was in homage to saxophonist Elton Dean and Blues singer Long John Baldry.
