I haven't had to go there yet but I've done some reading on it. Maybe start by doing a search on concierge medicine available by health care pros in your area. From what I've seen so far it looks to be about the same cost as the major insurance carriers.
Basically you bypass the insurance companies and pay the medical pros a retainer.
