How the heck did this happen 40 thousand times?

I'm guessing computer issues. The credit card theft is also computer issues... so I'm beginning to wonder if perhaps they've recently outsourced their IT operations to CGI (the outfit that "built" the Obamacare mess) or something.

So only a small percentage of them were processed incorrectly.

My question is, based on the sentence below from the article, if the wrong code was given out, how did it happen only 40,000 times? You know they had to sell more than 40,000 gift cards.

“Now the store is under fire for scanning the wrong code on gift cards when handing them over to shoppers, meaning that the value was not activated, KMSP in Minnesota reported.”
