There is a money making opportunity here for some smart docs who accept nO insurance but accept cash only. They could dominate enough of the market that they would be quite a force to reckon with.
By not allowing insurance they cut the expense of the overhead of a staff. Cash only is a huge market equalizer. Also allows a reduction of the costs of care and procedures that will allow market domination.
The lower prices would cause other doctors (insurance types) to try to compete throwing the market into a mini battle. Non-insurance docs who offer lower prices would really make obamacare patients want to not be on obamacare causing the younger and non heavy users to rebel.

What has also worked is single-office health maintenance plans. Anything that can be treated in the office is treated, for a flat monthly fee. No more insurance bookkeeping, just a list of the patients and the minimal billing overhead for the subscription.
