You don't pay the fine
Your account is flagged
Audit shows some minor discrepancy and you are cited for the discrepancy (almost every return contains one.)
IRS say payment for discrepancy and ObamaCare fine is due
They collect by any means necessary unless you fight them.
Suffice it to say, if you have ever had to fight with the IRS you know what a nightmare that is.
