If they don’t have health insurance then they have to pay the fine/penalty/tax to the IRS. It’s a no-win situation.
Pay cash and for big ticket things, go offshore. The prices will be much lower and the “procedures” will be much more likely available in a timely away. Research international health insurance for the big things. It is available and will become more so. Combine heart surgery with a nice hotel stay and a trip to somewhere in the Caribbean or Thailand.
