There can be some huge jurisdictional tiffs which are more important to the bureaucrat involved than people’s lives. Each service that shows is loaded with testosterone and starts ordering the others around. There may be little or no cooperation.

I’d say whoever is on the scene first should be the incident commander and all others would automatically subordinate, but I don’t think it happens that way because of the way they’re trained to work. Nobody is going to say their penis is smaller by taking orders.

I agree that at the point these decisions were made the presumption that the shooter was a jihadi was probably governing those making the decisions.
