The word you’re seeking is “physician”, or “surgeon”. There were “Doctors of Philosophy” (PhD) before physicians and surgeons were called “Doctor”.
I read somewhere the other day that physicians in the UK often don’t go by “doctor”, but I forget if that is nowadays or in days gone by.
