I am a B-777 pilot, based at SFO, a trained and experienced aircraft accident investigator, and I teach Crew Resource Manangement (CRM). I flew our plane into SFO last Wednesday. I am not involved in the investigation, but I have just looked at the crash site. I've flown into a great number of the airports involved in aircraft hull losses in the last 20 years. I am extremely familiar with the history of asian crews, particularly Korean crews, who defer the the Captain, mitigating all benefits of CRM, and resulting in a pilot caused crash. How about you?

It is unlikely, not likely. Too much communications about approach for that to happen.

You mean to tell us that a crew would use an inop ILS that far down to touchdown? Was the ILS transmitting?

P.S. What’s the decision height at SFO and why didn’t the pilot initiate a missed approach?
