I understand it is illegal in France, heard some French doctors setup shop in Northern Italy on the weekends and work for cash to keep food on their tables.
Back in the early 80's. I worked in Vancouver Canada. At that time, docs incomes were capped. If they had a good practice, by November they earned as much as they were allowed for a calendar year.
So they either practiced for free until January, or closed the doors for a month or two. Those who closed often would drive down to Bellingham WA and work the ER's. That income was not taxed in Canada (their tax rules) and often covered 50% of the gross they made in Canada in the 10 or 11 months they worked there.

Correction. Freepers need to learn that big gov and big business is the enemy of individual freedom. Insurance companies will tell gov to make direct care illegal. They will use the liberal wing of the population (and the liberal politicians) to vote against it.
Exactly what I was going to say. The Government will either make this illegal or the Insurance Companies will refuse them malpractice insurance at the request of Government.

Recall that the American Medical Association was all in favor of Obamacare. Like AARP, the association was run by liberals and they were co-opted by the Democrats early on.
They will lose hospital privileges.
And the AMA only represented about 17% of physicians - many of whom are academics. I suspect that percentage is lower now.

The American Medical Association was in favor of Obamacare, as you correctly noted. However, a majority of the physicians who are members
of the AMA were against it. The leadership of the AMA are cowards and
over ruled the membership.
