Perhaps sloppy work trying to get the correct spelling of the hospital... which they still goofed up.
Why would "sloppy work trying to get the correct spelling of the hospital" lead the alleged forger to split up the pre-printed line (which is unchanged in any version of the document) across multiple layers? And what about the other words that are similarly split. If the layers are evidence of intentional forgery (as opposed to artifacts of the scanning/etc process), then why would these anomalies exist? It does not make sense that a person constructing a document from layer would split up individual words (particularly words that are pre-printed on the form that is being forged) across multiple layers.
