If you don't count the several hundred tons of smallpox the Soviets produced during the cold war which our Government tells us has been (mostly) destroyed, or the remaining samples of live smallpox virus that were never destroyed (they were stored in 3 countries, can you guess them?), or the occasional case that pops up in some 3rd world rat hole, then yes small pox was eradicated.

I believe that, after the anthrax scare, the gov't (even under Bush) feared a terrorist group could acquire some of the smallpox virus kept for research purposes(?) and use it to start an epidemic.
