IMO, the decision to smoke tobacco is up to the individual. However, I also believe they should be prepared to accept and pay for the consequences of that decision. The taxpayer should not be saddled with the bill for smoking related health problems. I feel the same about alcohol use, and any other behavior where there are health risks.

But where does this assumption come from that all smokers receive public paid healthcare, while non-smokers don't? Are you really comfortable letting those in positions of authority dictate everyone’s behavior in relation to long-term health? That list of activities that might land someone in a hospital will get real long real quick.
