The only way it would be proved to be a fake is to investigate why the filing number is out of order.
It could just be that the paperwork from the hospital was sent at different times.

Second, I did not know they were using the word “African” instead of “Negro” in 1961?
Third, The signatures in box 18 and 19 look very similar.

candidly, if the white house was going to fake or alter this document, they would do a world class job of it. They would have the state gov of HI involved (the necessary people), and the only possible hole in the job would thus be plugged. They would not do a bush tx/ang dan-rather job.
