Just seems to odd, the only thing he might be embarrassed by would be it lists his mother as single. At least thats the way it looks. Where do you get that she is single? I see "single birth" checked (as opposed to twin or triplet). I see her maiden name in line 13 where it asks for her maiden name. But she signed with her married name.
