My certified birth certificate was issued by the county where I was born. The certificate of birth issued by the hospital was not good enough to get my U.S. passport.

Mine was signed by a famous criminal, Richard J. Daley, County Clerk of the County of Cook, in the State aforesaid, and Keeper of the Files and Records of said County.

My certified birth certificate was issued by the county where I was born.

When I had to get a security clearance the one issued by the State is what they wanted.

I just looked it over, it contains: My full name, and the city where I was born, and the date.

MY Father and Mothers full names,address and occupations and where they were born.

It has a seal, and where this info can be found (year-volume page and id#

It does not have the hospital or the attending physician on it.- tom
