As I recollect (from International Law class in 1961 one had to report to a federal official (of some sort, it might have been a Post Office...or it might have been Immigration & Naturalization) and, I would assume, present documents proving you were eligible for American citizenship.

And, I would imagine, present any passport or visa issued by the other country. Whereupon, I would again assume that you would be issued some kind of document (likely from Washington or some regional office).

Sorry for the vagueness...but it's been fifty years...
