I vaguely recall a problem with the F-16. There was a known
problem with wear on a lead to the artificial horizon, yet the
change order wasn't approved, even though GD brought up
the issue.

One pilot lost the instrument in low clouds, and given the speed and
low altitude, hit the ground.


Congress immediately passed a law holding manufacturers
responsible for defects, even if they were not authorized to fix
the problem[s].

It was on some doc channel about 10 years ago.


The last problem with F-22 that I heard about was the
software glitch that occurred when "passing" the International
Date Line.
