So what happens if the store worker simply catches a glimpse of the gun as the guy's shirt lifts up when he's reaching to get something off the shelf?

The premise of concealed carry is for the gun to be both hidden and immediately accessible. In order for it to be the latter, it is practically inevitable that at some time and in some place somebody will catch a passing glimpse of the gun tucked under a shirt or a jacket. It may not happen often and should not happen often, but it will happen.

And more importantly, the carrier does not deserve to be executed in cold blood simply because it happened.
