It looks like he has perfected his technique: Approach your face with your index finger, and at the last moment flick the middle finger. That really is what it looks like he did! A very intentional gesture on his part.

He does as stayathomemom says, definately and deliberately ending up with his middle finger. It’s an awkward motion, and wouldn’t seem to be accidental, especially given the corresponding dialog.
