Hands down most disturbing
I thought someone slipped me some LSD before starting to watch it.

There was a movie made a couple of years ago called “Funny Games” by an Austrian director (actually it was a scene for scene remake of his original German-language film) that is nothing but nihilistic sadism from beginning to end.
Somehow he got big name stars (Naomi Watts, Tim Roth) to be in it, though Roth was so disturbed by the final product that he refuses to list it on his resume.
I had to go stand outside at times to get some fresh air and recover. Don't know why I even finished it other than I had actually rented it and wondered if justice would be...um, served.
I remember seeing the movie Kids a very long time ago... I found that disturbing on a certain level. Event Horizon also disturbs me. Every time I see Sam Niell, I cringe.

Invaders from Mars, where the little boy’s parents turn against him and try to deliver him to the aliens.
I went to see the Nicolas Cage movie “8mm” when it was out in the theater and to this day I have never wanted to see it again. It left me a bit unsettled in the stomach when I walked out of that one.

Worst, most disturbing, sick, and depraved movie I ever made the mistake of seeing. It forever changed my opinion of Woody Harrelson as an actor and as a person. Sick.
