Mine has my mother and father's birthplaces, my father's occupation, my birth weight, number of living siblings, length of pregnancy, the date and result of my mother's test for syphilis, even the drug the doctor gave my mother to prevent ophthalmia neonatorum. And of course, the hospital and the signature of the doctor.
