Is that real? I knew she was incompetent as a lawyer, but can she not read and fill out a one page form?

I don't know if it's real but I've seen it posted on a couple of websites. I guess in today's world with photoshopping capabilities it is always best to be suspicious. One said it was posted on her blog or that's how I interpreted what they wrote but I don't go there.
